Advanced Practice Registered Nurses / Physician Assistants

Provide clinical expertise and program direction for medical services staff providing care in hospice in-patient units. Provide indirect supervision to medical staff to maximize quality of care provided to hospice in-patients while ensuring regulatory adherence. Assist medical and clinical services by setting and evaluating in-patient practice standards, providing in-patient care education, establishing quality assurance monitors, and ensuring excellence in hospice in-patient care. Analyze practices among hospice in-patient units and develop consistency in care provided to hospice patients in these settings.
Master’s degree in nursing
Licensure by state of Florida to work as an ARNP
ARNP certification recognized by CMS for Medicare billing
Current DEA license
Certification in Hospice and Palliative Care preferred or obtained within two years of hire date
Knowledge and experience in hospice in-patient medical practice and symptom management preferred
Advanced clinical skills which allow thorough patient assessments and management for any patient with serious illness as well as those near the end of life
Ability to work under protocol with multiple physicians in multiple settings
Ability to translate organizational goals into effective program implementation
Ability to provide effective education to clinical staff and serve as a mentor/preceptor for medical staff
Ability to monitor outcomes of program standards and implement change when needed
Strong interpersonal, communication (written and verbal), collaboration and conflict resolution skills
Ability to communicate with community physicians regarding patient treatment options and program standards
Ability to work independently with other disciplines within the Organization, exercising discretion and independent judgment
Valid driver's license and automobile insurance per Company policy
Ability to travel to multiple locations with reliable transportation
Ability to manage multiple priorities simultaneously and effectively handle the emotional stress of the workload

Hospice In-patient Care
Evaluate and manage patients in in-patient settings while optimizing their care.
Serve as an expert in the provision of in-patient care to hospice house patients and as a mentor to hospice house medical staff.
Assist hospice house staff in making treatment and care recommendations regarding complicated hospice house in-patients, as needed.
Collaborate with the patients’ hospice and/or community attending/consulting physicians, as needed, to assist patient/family in making treatment decisions that are in their best interest.
Respond to clinical in-patient emergencies, as an expert consultant, to medical and nursing staff, as requested.
Collaborate with Hospice House/Clinical Managers and medical staff to identify the needs of patients, families and staff regarding hospice house in-patient level of care.
Maintain practice excellence through active involvement in direct patient care, as requested.
Utilize practice opportunities to provide expert care to patients, to assist in evaluating clinical practice, and to provide clinical learning activities for medical and nursing staff.
Act as a role model for advanced practice by integrating all components of the professional role: clinical practice, consultation, collaboration, leadership, education, and research /evaluation.
Provide consultation to nursing and medical staff, Hospice House and Clinical Managers, and other health care professionals regarding in-patient care of hospice patients.
Participate in hospice house complex case conferences, as a consultant, to assist in identifying best options for care management of patients in the hospice house setting.
Collaborate with hospice and community health care providers, as appropriate, to define Organizational practice standards and outline appropriate case management.
Implement program policies and procedures that maximize appropriate utilization of and access to in-patient services in hospice house settings.
Develop and implement hospice in-patient program standards and protocols to ensure high quality care to patients in those settings.
Collaborate with appropriate departments within the hospice affiliates to develop effective hospice in-patient unit patient care practices.
Identify and implement best and efficient practices and ensure consistency for the hospice house interdisciplinary team meetings.
Evaluate the effectiveness of current practices and processes in the hospice house in-patient settings and make recommendations for improvement/efficiencies.
Evaluate the Organizational use of hospice in-patient services, develop and retrieve appropriate outcome measures, and develop programs to maximize its effective use.
Work with Compliance Department and affiliate hospice to monitor adherence to regulatory requirements regarding hospice in-patient level of care.
Implement program and monitoring for documentation excellence in the care of patients in the in-patient setting.
Assist the hospice house and clinical leadership in the evaluation of affiliate practices regarding the appropriate use of in-patient level of care services.
Collaborate with other departments/leadership when recommended changes in practice are indicated.
Monitor the Company’s costs of hospice house in-patient care, including medications, laboratory and diagnostic testing, and develop/implement cost savings or efficiency changes, when appropriate.
Develop and provide educational programs on Organizational approach to utilization of hospice house in-patient services.
Make recommendations for and participate in orientation program, as appropriate, regarding hospice house in-patient level of care and Organizational approach to best practices in these settings.
Develop and implement in-patient orientation for new medical staff.
Assist in the development of the competency program related to hospice house in-patient medical staff practices.
Serve as a preceptor for new medical staff to ensure competent medical practice in the provision of in-patient level of care services.
Provide Medical Services and Hospice House Managers with feedback regarding strengths and opportunities for development regarding hospice house in-patient care.
Develop appropriate educational tools to facilitate learning.
Provide education for Professional Relations Representatives and Directors so they may educate community physicians and other appropriate persons on Organizational approach to utilization of hospice houses for in-patient level of care.
Research and Development
Support and assist in implementation of hospice house projects to promote clinical knowledge, development and research-based clinical practice.
Conduct research and submit for presentation/publication, as appropriate.
Assume responsibility as a self-directed professional for own continuing education.
Identify needs or seek direction in identification of needs to maintain expertise and keep current with hospice in-patient health care trends and practices.
Participate in professional organizations to advance the profession and improve hospice house in-patient health care delivery to patients.
Perform other duties as assigned.
