Provides individual, family and group counseling to patients and their families.
Completes psychosocial assessments and participates in interdisciplinary treatment planning, community linkage and is primarily responsible for discharge planning with timely and accurate documentation of all patient care activities and/or advocacy.
Can identify and work through sources of client's resistance to treatment.
Completes an age appropriate psychosocial assessment.
Demonstrates the ability to identify and facilitate groups that are clinically appropriate to the current client population.
Demonstrates the ability to utilize appropriate interventions in both non-crisis and crisis situations.
Develops formulation of the case incorporating presenting problem, and current life circumstances.
Documents in a clear and precise manner progress notes for all patient contacts.
Initiates discharge plan at time of admission with the multidisciplinary team, patient, family, school and other available resources
Is able to explore alternative treatment modalities and make referrals when appropriate.
Is able to identify when treatment goals have been reached.
