This physician will join Community Physician Network, an employed physician group of over 600 physicians in Indianapolis, Indiana and Fishers, Indiana.
Community Physician Network is seeking a Hospice and Palliative Medicine Physician to join our outstanding practice. This full-time position will be a mix of Hospice (60%) and Palliative Medicine (40%), providing patient care and consultative services with Community Home Health Services and at four of our Indianapolis-area hospitals (Community Hospitals North, East and South, and Community Heart and Vascular Hospital). This position will report to the Medical Director of Palliative Care and Home Health.

Community Home Health, based in Fishers, Indiana near Indianapolis, has been providing home care to residents of central Indiana for more than 55 years. Community Home Health's hospice services are provided by an interdisciplinary team of skilled professionals and carefully selected and trained volunteers. The team works with the patient and family to meet their unique, changing needs.
Our Palliative Care program is focused on education, decision making and symptom management. It is a support-based program intended for people facing complex symptoms, pain, psychological, and spiritual issues. Through this unique program, the most comprehensive of its kind in Indiana, improved coordination of care and better management of symptoms is possible. We can make a meaningful difference in how an illness is managed, so that patients can get back to focusing on what’s important in their lives.
Licensure & Certification
Board Eligible or Board Certified in Hospice and Palliative Medicine and eligible for medical licensure in Indiana.
