Providence is calling a Social Worker - Care Manager (Innovation Clinic, Days, Full time - 1.0 FTE) to Providence Medical Group in Spokane Valley, Washington. We are a community of caregivers delivering every day on our Mission to provide compassionate care that is accessible for all - especially those who are poor and vulnerable.
In this brand new position you will:
Support and closely coordinate mental health care with the patient's primary care provider and, when appropriate, other mental health providers
Screen and access patients for common mental health and substance abuse disorders
Monitor patients (in person and phone) for changes in clinical symptoms and treatment side effects or complications
Provide brief interventions using evidence-based techniques such as Behavioral Activation, Problem solving treatment, Motivational interviewing or other treatments appropriate for primacy care settings
Participate in regularly scheduled caseload consultation with the psychiatric consultant and communiate resulting treatment recommendations to the patient's PCP. Consultations will focus on patients new to treatment or who are not improving as expected.
Required qualifications for this position include:
Master's Degree in Social Work (MSW) Licensed as a Social Worker ("Advanced" or "Independent Clinical") in the State of Washington (both are LW credentials) or licensed as a Counselor in the State of Washington while completing supervision requirements toward the Social Worker license
Basic Life Support (BLS) certification within 90 days of hire
2 to 4 years of experience in the field or in a related area
Able to listen, read, speak and write using English language
Preferred qualifications for this position include:
State license to practice clinical social work
LICSW is preferred (required to obtain within one year of hire)
Prefer two years experience in an ambulatory health care setting
Experience working with patients who have co-occurring mental health, substance abuse and physical health problems
