We are currently seeking Social Workers or Psychologists to provide care to patients ranging in age from child through geriatric. Along with serving as a clinical liaison between Psychiatrists, RNs and ER Attending, you will provide mental health screening assessments, make appropriate interventions and referrals, and provide a full range of mental health treatment services 24/7, including: Mobile Outreach/Screening, responding to hotline calls, assessments, crisis counseling, case management, as well as participating in interdisciplinary treatment planning, record keeping, consultation and education.
Performing comprehensive clinical assessments and interpreting data to formulate a DSM IV TR diagnosis.
Collaborating in the formulation of treatment/service/discharge plans with a multi-disciplinary team.
Recognizing, evaluating, gathering collateral information and developing interventions to address risk factors related to potential for violence and dangerousness to self or others.
Providing crisis intervention/stabilization and aggressive behavior management to consumers in a safe, culturally sensitive and gender specific manner for patients in our care, both on-site and off-site.
Providing clinical case management services, including but not limited to: assessment, referral and linkage to medical and community services, follow-up, outreach and coordination of service delivery.
Documenting services provided to consumers and maintaining medical records in accordance with established governing standards in a timely, legible, accurate and complete manner. This includes, completion of log book entries, USTF's, and patient follow-up.
Providing hotline coverage and cover telemedicine services.
Conducting mobile outreach and screening of consumers who may be in need of commitment, in consultation with the on-call/on-site Psychiatrist and Shift Supervisor.
Providing consultations, training and technical assistance to other community providers, municipalities and family members.
Performing all other duties as assigned, including special projects.
To qualify, you must have a Masters’ degree in Social Work, Psychology or closely related field and one year experience in a psychiatric setting, or, a Bachelor's degree and two to three years of previous experience. Valid driver’s license is essential, as is the analytical ability to collect information from diverse sources, apply professional principles in performing various analyses, and summarize the information and data in order to effectively solve problems. Candidates must be able to prepare detailed assessments of patient referrals, counsel individual, group or family members, and/or perform other duties requiring a comparable level of communication skill.
