The Wyoming Life Resource Center is located in central Wyoming and serves individuals with intellectual disabilities, mental health issues and brain injury. This position serves as Social Worker for the Skilled Nursing Facility (SNF) at the Wyoming Life Resource Center in Lander, WY. It also serves as Admissions and Discharge Coordinator for both our Skilled Nursing Facility and Intermediate Care Facility (ICF).
Job Duties for this position are as follows:
Work with the interdisciplinary team and administration to promote and protect resident rights and the psychosocial well-being of each resident.
Work with resident families and/or guardians to provide support, information and education.
Clinically assess the resident's mental health and complete assessments that identify social, emotional and psychological needs.
Participate in the resident care planning process in developing an interdisciplinary plan of care for each resident.
Monitor and ensure that resident psychosocial needs are being met, including treatment by staff, resident to resident interactions and opportunity for community activities.
Facilitate clinical interventions to address catastrophic events that occur during the resident's stay at the facility.
Oversee the creation and implementation of all policies and procedures for admissions and discharges, to include both the ICF and SNF and ensure compliance with internal policies and federal licensing regulations.
Coordinate all appropriate admission and discharge procedures with clients, guardians, multidisciplinary teams and outside agencies to ensure all plans are in place at the time of admission and discharge for these individuals.
Work with the team to identify appropriate discharge criteria and community supports for residents. Maintain accurate records and reporting.
Work with all stakeholders, such as the Wyoming State Hospital, community programs and the Wyoming Department of Health to determine eligibility and proper placement for persons served.
Participate in strategic planning to meet present and future needs.
