Northeast Georgia Health System (NGHS) announces an exceptional opportunity for an accomplished nursing leader to serve in the newly created role of Executive Director of Care Coordination. Anchored by Northeast Georgia Medical Center's three hospital campuses – NGMC Gainesville, NGMC Braselton and NGMC Barrow – with a total of 713 beds and more than 700 medical staff members representing more than 50 specialties, the health system provides care to more than a million people in more than 18 counties across Northeast Georgia.
Reporting to the Vice President of Revenue Cycle, the Executive Director of Care Coordination maintains system-wide responsibility for aligning the clinical, administrative and financial functions related to the provision of integrated care management across the spectrum of care to achieve the highest level of clinical and financial outcomes. Providing leadership to a team of 164 full-time employees, including four direct reports, the Executive Director oversees the planning, direction and implementation of roles, processes and programs for care coordination, case management, health information management, social work services, utilization and payor management, readmissions reduction, transitions of care, and clinical documentation improvement. In addition, the Executive Director will establish strategic goals for growth, sustainability and compliance, creating standardization and structure across the health system for achieving financial sustainability and revenue cycle efficiency, clinical and operational excellence and patient throughput and safety.
The successful candidate will demonstrate a minimum of 10 years’ progressive leadership experience in a hospital or health system, including at least five years’ directing a continuum of care or integrated case management department. A Master’s degree in a health care related program, nursing or social work, as well as current Case Management Certification or American Case Management Certification, are strongly preferred. Excellent leadership qualities, communication skills, and the ability to establish strong alignment with physicians, nursing, members of the multidisciplinary care team and administration are required.
