RN Care Coordinator, Case Management Department (FT days)
Located in Belleville, Clara Maass Medical Center is a facility of RWJBarnabas Health and provides a complete continuum of care to residents of northern New Jersey. With over 700 physicians and 2,100 employees, we offer advanced treatment for a wide variety of medical conditions. Our facility includes centers for advanced wound care, sleep disorders, bariatrics, orthopedics cardiac services, cancer care, emergency services, and more. In 2017, the Medical Center completed a state-of-the-art campus expansion featuring a new four-story 87,000 square foot building with a private, 32-bed Intensive Care Unit, new Main Entrance and Lobby, Retail Pharmacy, Patient Registration Area and designated space to expand its Emergency Department, which is currently underway.

We are currently looking for a full time RN Care Coordinator . Under the direction of the Director of the Case Management Department, this position provides care and services of Clara Maass Medical Center patient population across the continuum of illness.
You Will Be Responsible For:
Coordinates the care and services of Clara Maass Medical Center patient population across the continuum of illness.
Assumes responsibility for utilization management and monitoring process.
Conducts admission and continued stay review on groups of patients, in accordance with the PRO, special negotiated PPO and/or HMO contracts, and on other classifications of patients as designated by the Utilization/Case Management Review Plan.
Coordinates patient care, in conjunction with the medical, nursing and other ancillary staff members.
Identifies trends and patterns of resource use and patient care, and makes recommendations in compliance with established policy and procedure.
Ensures continuity of care for those patients with post-hospital health care needs and maximizes the patient s and family s abilities to meet the continuing care needs within their social and financial framework.
Uses an understanding of principles of growth and development to assess each patient s age specific needs and provide age specific treatment and care.

Your Qualifications Are:
Registered Nurse with a baccalaureate degree from an accredited school or university preferred, or equivalent RN degree with commensurate clinical experience.
Minimum of 2-3 years experience in a hospital setting.
Minimum of one year experience in Case Management/Utilization Management or Home Care.
