JOB TITLE: Nurse Practitioner Physician Assistant ? Pulmonary/Sleep Medicine LOCATION: Geisinger Jersey Shore - Jersey Shore, PA DEPARTMENT: Pulmonary/Sleep Medicine WORK SCHEDULE: Days WORK TYPE: Full time JOB DESCRIPTION: Serves in a Nurse Practitioner/Physician Assistant role as defined by state guidelines, providing comprehensive assessment and care delivery, demonstrating a high level of autonomy and expert skills in the diagnosis and management of patients. Serves in a collaborative practice with the department physician staff in assessing needs, planning and providing education and care, evaluating outcomes, and coordinating follow-up and referrals for patients. Meets the qualifications to provide care for patients in the specific age range in both the Pulmonary and Sleep Medicine Clinics.
CLINICIAN: 1. Provides direct care including assessing, diagnosing, planning and prescribing pharmacological and non-pharmacological treatment, health promotion and preventive care within the scope of the department practice. Provides comprehensive clinical coordination of care and case management. 2. Provides interval follow-up care for those already established in the panel of Sleep Medicine Physicians and Pulmonologists to evaluate patients requiring short-term return visits (i.e. evaluation of asthma patients to assess the efficacy of new medications, etc.). 3. Collects comprehensive health data. 4. Identifies expected outcomes and analyzes results. 5. Develops a comprehensive plan of care that includes interventions, treatments and education to attain expected outcomes. 6. Prescribes, orders and implements treatment strategies and self-care management and education within the scope of advanced practice. 7. Participates in educational programs for patients with chronic lung disease.
EDUCATOR: 1. Employs complex education strategies and interventions to promote, maintain and improve health and prevent illness and injury. 2. Serves as a resource person for patient, family and health care staff. 3. Provides educational opportunities for nursing and medical health professionals.
CONSULTANT: 1. Provides consultation to medical and nursing staff. 2. Collaborates with other health care professionals to identify and resolve patient care issues. 3. Offers specialized skills and knowledge to assist with problem solving. 4. Acts as a liaison with other advanced health care practitioners and providers in other facilities.
COMPETENCIES & SKILLS: Demonstrates ability to identify problems and needs of patients; carry out defined responsibilities; relate to patients, families and outside agencies; establish rapport with physicians and co-workers; maintain records; and demonstrate growth in the position and an interest in self development and expansion of knowledge.
EDUCATION/EXPERIENCE: Must be a graduate of an ARC approved physician assistant program, eligible or certified by the NCCPA and eligible or licensed by the Pennsylvania State Board of Medicine. Or certificate of completion from an approved program for Nurse Practitioners and completion of Master's degree required. Must be eligible for licensure as a nurse practitioner in the state of PA. Current certification in BLS and an updated CPR card required. Degree of professional training to qualify as a physician assistant/nurse practitioner both professionally and legally to perform all services indicated by the Pennsylvania State Board of Nursing and licensure as described in the policy and procedure manual for utilization of physician assistants/nurse practitioners within the Geisinger System required. Completion of an approved graduate program required. Completion of an approved ACLS program preferred.
