As our most acute level of care, Kindred's transitional care hospitals offer the same critical care patients receive in a traditional hospital or intensive care unit , but for an extended recovery period. Our clinicians play a vital role in the recovery process for chronic, critically ill and medically complex patients .
Oversees and leads the operations of the Centralized Admissions office including the supervision of the Admissions Coordinators. Serves as a liaison with marketing, business office and administration of the facilities to assure timely processing of admissions and to assure total customer satisfaction. Assures coordination of admissions with other departments by facilitating the collection and distribution of appropriate information.
Essential Functions :
Supervises staff assigned to centralized admissions function.
Liaison with marketing, business office and facilities' administration to assure timely processing of admissions and to assure total customer satisfaction
Oversees coordination of and facilitates the admission of patients from referral sources. Assists in completing the Pre-admission Clinical Evaluation (PACE) form to assist in evaluating appropriateness of admission. Applies appropriate clinical guidelines and risk tolerance (as needed) to pre-admission data.
Communicates referral. Notifies clinical liaison or designee of admission decision. Assists with coordination of patient transfer. Facilitates and coordinates physician-to-physician communication as necessary. Identifies and communicates specific patient needs for equipment, supplies, Pharmacy and consult services. Provides hospital team with needed information on pending/new admissions.
Develops the necessary admission and patient day forecast for the region in conjunction with the healthcare team.
Develops appropriate analysis, using established reporting mechanisms and provides data and statistics as required, within the established time frames.
Maintains a close working relationship with Medical Staff, Clinical Liaisons, interdisciplinary healthcare teams, and with external professional groups and agencies.
Communicates with Medical Advisors of insurance company as needed.
Manages the MANG application process to assure all necessary information if collected, facilities are updated on MANG status and troubleshoots as necessary.
Bachelor's degree in clinical area preferred.
Minimum of three years' experience in case management, admissions office management or similar leadership capacity.
