Position Summary: Continuum of Care is seeking an individual to provide comprehensive planning and actively support effective implementation and operations for our CRS treatment program.
In delivering the highest level of care and treatment this role will coordinate the development, scheduling, and evaluation of assigned program(s). The individual will provide oversight for the case management and recovery-based services and networks closely with area community providers and families. Models self-care and wellness for staff and clients. Interviews, trains, and provides professional development to staff, interns, and volunteers for the positions which it supervises.EOE
Essential Job Functions:
Assists Program Director to ensure effective administration of program standards and procedures related to the operation and provision of services.
Assists in monitoring costs and allocation of resources for program, in accordance with agency and client needs; includes monitoring of client finances and budgets as well.
Ensures compliance with applicable contractual, licensing and accreditation requirements.
Implements changes in program reflecting identified agency priorities and strategic plan.
Maintains consistent and positive working relationship with families and identified community providers.
Ensures recovery-based, person-centered curricula and service delivery are designed and facilitated daily.
Fields calls from potential referral sources and screens clients including meeting with families, either in person or with the referral source.
Supervises program admissions, planning, and discharge processes and works closely with Outreach Coordinator to monitor referrals, capacities, and waitlists as applicable.
Ensure all service documentation and data submission is maintained and processed timely consistent with insurance billing, contracting, and Agency requirements.
Attends agency and external sponsored trainings and meetings for continued professional and agency growth.
Participates in program development activities and sees to their implementations; educate clients and staff in how to meet community support plan goals and objectives.
Ensures company vehicles are in good working condition and completes periodic maintenance inspections work orders as necessary. Works with team to maintain vehicle cleanliness inside and outside at all times.
Ensures continuity of care for delivery of services.
Provides direct clinical, crisis, and residential supports to staff regarding client care in collaboration with and direction from Program Director.
Schedules, attends and participates in agency, staff and team, and inter-disciplinary team meetings.
Master’s degree required with a minimum of three (3) years’ consecutive managerial experience. Licensed Preferred, related behavioral health field (LPC, LCSW, LMFT) exception of LMSW.
Demonstrates an understanding of the recovery process.
Prior experience working with people with mental illness and substance use disorders is required.
Valid Connecticut Driver’s license is required. Reliable transportation and willingness to transport clients in personal liability insured vehicles.
