Provides support in coordinating and executing family navigation services. The family navigator program involves physician and care provider education, early education, and telemedicine to improve care coordination for children with autism spectrum disorder and developmental delays across medically underserved areas of Tennessee.
KEY FUNCTIONS ( The responsibilities listed are a general overview. Additional duties may be assigned.)
Provide support and resources to families, physicians and community service providers
In a very timely manner, provide support and resources over the phone to individuals (families, physicians, service providers) who call the central resource phone line
Respond to emails that are received on the central resource email account
Keep accurate and up to date data on all calls and emails
Address family needs related to early screening concerns, evaluation and diagnosis, service access, engagement with educational systems, and common behavioral challenges.
Support families to increase knowledge and ability to be a self-advocate.
Build relationships with service providers at VUMC as well as in the community in order to serve as an effective liaison. Close loops between referral sources and families to create comprehensive systems of care.
Assist in investigating and collating resources for patient population
Research potential resources for individuals with autism spectrum disorder and developmental delays in underserved areas
Organize and collate resource information
Distribute resources to community service providers and physicians
Collect and appropriately document information required for deliverables
Maintain appropriate documentation required of the organization and funding sources
Support patient appointments
Ensure that the appropriate equipment is available, set up, and in working order for appointments
Collect insurance information as needed for appropriate billing
Document information in medical records
Provide administrative support to psychologists during patient visits
Provide general administrative support for the office
a. Reserve rooms, parking spaces, etc. for meetings+
b. General office responsibilities such as faxing, copying, completing errands
* Bachelor's Degree (or equivalent experience) and <1 years experience
*** Preferably a degree in early childhood education, special education, social work, psychology, or other helping profession,1 year of experience preferred but not required
