The Social Worker is assigned to the Care Management and Social Work Service (CMSWS) of the Amarillo VA Health Care System (AVAHCS). The social worker functions as the Patient Care Team (PACT) and Transition Care Management (TCM) Social Worker, utilizing skills in assessing and treating complex psychosocial problems, often at a time when the Veteran is experiencing crisis and includes the provision of short-term, solution-focused counseling with Veterans, their family members, and/or caregivers.
