| Provides an integrated continuum of care for patients from preadmission to post-discharge. Screens for discharge planning needs and initiates a plan for community resources and/or placement in appropriate post-discharge facility. Serves on committees as requested.

Education/Other Qualifications:
Minimum Education: BSW required; Master�s degree preferred.
Minimum Experience: 1 to 2 years of job-related experience preferred, including experience as a licensed clinical social worker, experience in an acute care setting, knowledge of federal and state laws and Medicare/ Medicaid calculations, and experience with computer software.
