The Senior Social Worker practices in the Health Care for Homeless Veterans (HCHV) Program and serves a patient population with highly complex health and mental health problems. The incumbent serves as a case manager in the VA Supported Housing (VASH) program. The HCHV program may be based at a Community Based Outpatient Clinic (CBOC) or the main Medical Center, but social workers provide services to clients primarily in the community. The primary responsibilities of this position include clinical and administrative program oversight and monitoring; serves as one of the primary contacts with the local Public Housing Authority's (PHA's); provides clinical and psychosocial case management services; and ensures that the Veteran's care is coordinated with the VA Medical Center, community health or mental health providers, and agencies that serve homeless populations. The Senior Social Worker in this position will utilize various treatment modalities of individual and group work, casework, community organization, and community outreach; participate in program planning and development, and process improvement activities; and participate in annual community based events. The Senior Social Worker implements, and maintains referral and screening procedures for potential VASH Veteran participants that meet national policy as well as the needs of medical center mental health services and residential programs. Referral, screening and admission criteria recognizes program eligibility, requirements of the PHA's and other policy, regulation or law pertaining to administration of the program. The incumbent provides primary input for program participant selection based on structured interviews and clinical assessments.
