Description JOB SUMMARY: Provides psychosocial assessment of and brief targeted intervention for social, emotional and environmental issues that impact client/family's ability to optimally benefit from care. Actively collaborates with team to meet patient care management and system improvement goals. Provides leadership for department committees/workgroups. Contributes to the plan of care with knowledge of population specific needs as it relates to the identification of patient psychosocial needs. Department/Unit Summary: Welcome to the Vanderbilt Weight Loss Center. The clinic is located at Vanderbilt Health at One Hundred Oaks, 719 Thompson Lane Suite 22000 Nashville, TN 37204. We provide both medical and surgical weight loss for the adult population. We are looking for highly motivated individuals that would like to join a growing clinic. Our excellent, patient centered care is directed and/or delivered by a collaborative team made up of Attending Physicians, Nurse Practitioners, Dietitians and Psychologists, Exercise physiologist and program coordinator. The Leadership Team is comprised of a Medical Director, a Clinic Manager, an Administrative Manager, a Charge Nurse and an Administrative Assistant. Clinic staff members include Registered Nurses, Licensed Practical Nurses, Medical Assistants, and Patient Services Representatives. Various support resources are also available including social work, referral specialists, laboratory and radiology.
