The Greater Los Angles Healthcare Network is recruiting for one (1) Senior Social Worker located in the Santa Barbara Community-Based Outpatient Clinic, on the Patient-Aligned Care Team (PACT). The incumbent functions as a Case Manager and Care Coordinator for two (2) primary care panels. The incumbent is licensed at the advance practice level providing services to Veterans in Primary Care. The focus is on prevention, health promotion, coordination and chronic disease management. The Senior Social Worker is a member of the PACT team, providing mental health diagnoses, treatment procedures, including acute, chronic and traumatic illnesses, and substance abuse disorders. The major duties and responsibilities include but are not limited to: Provides assessment of psychosocial problems that cause distress, such as adjustment to the current medical condition and/or functional status, untreated or under-treated mental health or substance abuse condition, economic instability, legal problems, inadequate social supports, housing, and transportation. Provides direct psychosocial services in the form of crisis intervention, individual counseling and psychotherapy, group counseling and psychotherapy, and family counseling. Assesses and treats the complicated psychosocial problems of Veterans, families and significant other. Provides consultation and education to Veterans and their families regarding community resources, VA benefits and specialty programs. Engages in case management services with members of the outpatient interdisciplinary team to meet the agreed upon treatment goals. Develops and implements methods for measuring effectiveness of social work practice and services in specialty areas, utilizing outcome evaluations to improve treatment services. Provides on and off site crisis intervention and outreach (individual sessions, assist and coordinate hospitalizations in conjunction with the physician when required, coordination of treatment with collaterals (family, housing programs, nursing homes, mental health etc.) to maximize the successful stabilization and integration of Veterans in the community both in crisis and non-crisis situations. Provides intervention to increase adherence to treatment plans and access to care, transportation assistance, Advance Directive education, economic assistance and strategies to increase income, assist in locating housing resources to assist with functional decline. Determines the frequency and intensity of case management interventions for preventative treatment, and treatment of acute and chronic illnesses, end of life assistance and to maximize health outcomes.
