This Senior Social Worker position is located at the Edward Hines Jr. VA Hospital in Hines, IL and is part of the VA's Great Lakes Health Care System in the Veteran's Integrated Service Network (VISN 12). The senior social worker assigned to the Emergency Department has a responsibility to provide direct services to veterans and their families presenting for services in this area in conjunction with necessary follow up to inpatient areas. Major Duties: Responsible to provide a variety of services to veterans referred from the Emergency Department including psychosocial assessments, treatment planning, short-term, solution focused counseling with individuals and families who may be in crisis. Conducts psychosocial assessments to determine needs of veterans and family members. Participates in the treatment planning process with other disciplines. Assists in discharge planning with other disciplines and will ensure discharge plans are executed in a manner that is timely and appropriate. Acts as liaison between VA and community resources, to include marketing and public relation duties as needed. Addresses advance directives and organ donation in accordance with the standards of their work area. Supervises the practice of social work students, or a less experienced social worker, as appropriate. Maintains data and statistical compilations to comply with JCAHO, VHA, medical center policy, and procedures. Documents social work interventions in veteran's clinical record utilizing CPRS, and ensure appropriate sign out when transferring patient care. Initiates and completes their initial assessments to assist in timely provision of emergency services to assist with patient flow in the Emergency Department.
