The Social Worker will provide a range of direct patient care services consistent with area of assignment. Demonstrates the ability to balance quantity and quality requirements, agreed upon priorities are maintained, and case load is effectively managed, with evidence of acceptable work quality by satisfactory supervisory or peer case review ratings and not more than one documented justified complaint/observation of errors in professional procedure/judgments during the rating period. a. The Social Worker will provide a range of direct patient care services consistent with area of assignment. The Social Worker demonstrates the ability to balance quantity and quality requirements, agreed upon priorities are maintained, and caseload is effectively managed, with evidence of acceptable work quality by satisfactory supervisory or peer case review ratings and not more than one documented justified complaint/observation of errors in professional procedure/judgments during the rating period. b. The Social Worker engages in psychosocial/diagnostic assessment, establishes therapeutic rapport, administers therapeutic interventions, and ensures continuity of patient care in the development and implementation of the psychosocial plan, including expected short and long range outcomes, with clients of all ages, with evidence of acceptable work quality by satisfactory supervisory or peer case review ratings and not more than one documented justified complaint/observation of errors in professional procedure/judgments during the rating period. The Social Worker is to establish a proactive, resourceful approach to discharge planning. This includes initiating, coordination, and expediting the successful discharge of Veteran clients. Demonstrates the ability to perform and monitor quality professional psychosocial services for the young, middle age, and geriatric patients. c. Effectively communicates with and utilizes community agencies to facilitate treatment, discharge planning, and continuity of care. Demonstrates regular contact and interaction with a variety of community agencies and resources. Collaborates with a wide variety of community agencies in a manner that enhances coordination of comprehensive patient care, and/or problem resolution. d. Effectively communicates with and utilizes other professions to facilitate treatment planning and implementation. Participates effectively in team meetings, unit rounds, treatment planning conferences, etc. Collaborates with divergent multidisciplinary team members in a manner that enhances coordination of comprehensive patient care as indicated by no more than one documented justified complaint/observation demonstrating an inability to work effectively. e. is able to coordinate community-based services through information and referrals to community agencies, VA, and other government programs as needed. f. Completes suicide screening and related clinical reminders to identify high-risk patients and make referrals for additional services when indicated. Incumbent provides crisis intervention services, seeking to address the cause as well as the presenting complaint and has a working knowledge and experience in use of medical and mental health diagnosis, disabilities and treatment procedures. g. is able to communicate effectively, both orally and in writing, with people from varied backgrounds. h. has general knowledge of veterans' benefits and services related to special programs, service connected compensation, and non-service connected pension and facilitates referrals based upon veterans' needs and eligibility. i.Has working knowledge and skill in the use of word-processing, data management and other VA computer systems, especially those programs in use by VHA. j. Independently completes all required VA trainings on LMS and VA Intranet as well as those required to maintain an independent social work licensure.
