The Health Care For Homeless Veterans (HCHV) Program is funded by the Department of Veterans Affairs and authorizes the provision of outreach services, assessment, health care, residential treatment, and referrals for VA/community services to eligible Veterans. The objective of the HCHV Program is to re-integrate homeless veterans with complex psychosocial issues into the community and to help them achieve the maximum level of health and social functioning as possible. The HCHV Program Coordinator supervises a significant number of professional staff at grades ranging from GS-05 through the GS-13 level. These positions include senior social workers, social work program coordinators, social work supervisors, occupational therapists, nursing staff, peer support staff, administrative staff, employment specialists, and other staff assigned to HCHV. The Social Worker Program Coordinator assigned to manage the HCHV Program is responsible for the professional and administrative management of the Program across multiple sites and programs; provides direct clinical services, must be licensed at the advanced practice level and must be able to provide supervision for subordinate staff to qualify for independent licensure. Duties include but are not limited to: Assists in the planning, establishing and implementation of policies and procedures. Develops p individual or group goals and objectives as well as monitoring, operating, evaluating, coordinating and overseeing staff assigned to the HCHV Programs. Ensures the staff whom they supervise adhere to all current directives of the VHA, and meet current JCAHO and CARF standards, State licensure standards and VA-OIG expectations Conducts ongoing intensive reviews and appraisals to ensure the accomplishment of objectives and adherence to standards set by the governing bodies. This position is assigned to Social Work Service. The Health Care for HCHV Program Coordinator is an integral member of the facility's social work leadership team, and is responsible for the following Programs. These Programs include: the Veterans Outreach Program (VOP), Housing and Urban Development/VA Supported Housing (HUD/VASH), State and Federal Stand-downs for Homeless Veterans, Contract Residential Programs, Grant and Per Diem (GPD), Homeless Patient Aligned Care Team (HPACT), and the Veterans Justice Outreach (VJO) Program. The Programs under the auspices of HCHV are available across the NMVAHCS catchment area, to include the State of New Mexico, identified Tribal Lands, and areas in southern Colorado.
