The VISN 10 Clinical Resource Hub is recruiting for four Social Workers (Patient Aligned Care Team). The Social Workers will function virtually within the VISN 10 Clinical Resource Hub, a repository of clinical and administrative staff that serve VA facilities within VISN 10 that are underserved or experiencing gaps in clinicians due to the inability to match provider supply with demand. Services are conducted via multiple modalities, including virtual and on very rare occasion, in person. Assesses and treats psychosocial problems of Veterans, families, and caregivers. Performs assessment of psychosocial issues, including those that impact health conditions and create barriers to care. Participates as a member of the interdisciplinary treatment team, which includes Veterans, families, and caregivers in the development and implementation of treatment goals and interventions. Uses appropriate medical and mental health diagnoses, terminology, disability definitions, and treatment procedures, including acute, chronic and traumatic illnesses, substance use disorders, and common medications and their effects/side effects. Provides psychosocial education to Veterans, families and caregivers regarding identified needs, discusses the benefits and risks of possible interventions, and refers to appropriate services. Acts as a liaison with VA staff and community agencies to meet identified needs, such as: programs to increase access to care, economic assistance, housing assistance and coordination of appropriate services. Establishes and maintains effective therapeutic relationships with Veterans, their families, and caregivers in the PACT and works independently with those who are experiencing a wide range of complicated medical, mental health, financial, legal and psychosocial challenges. Facilitates community placements through collaboration with Veterans, families, caregivers and the interdisciplinary treatment team members to ensure that appropriate placements are completed in a timely manner. Implements treatment modalities. Provides educational classes and/or supportive groups for Veterans, families, and caregivers including shared medical appointments. Instructs Veterans in the effective use of the secure messaging system and various telehealth treatment modalities. Provides consultation and education to Veterans, families, and caregivers regarding community resources, VA benefits and special programs, and advance directives. Maintains knowledge of Veteran's benefits and services, community resources and processes for making appropriate referrals to community and other governmental programs or agencies. Serves as a liaison between VA and community resources in order to ensure appropriate delivery of services, including outreach activities when needed. Provides proactive outreach and ongoing follow up services to Veterans throughout the continuum of care. Assists Veterans in coping with the loss and grief experiences associated with chronic medical and/or mental health conditions, disability, and terminal illness. Serves on committees, work groups, and task forces at the facility, CRH, VISN, or national level as deemed appropriate by supervisor. Contributes to data collection relevant to the program regarding day-to-day functioning and program evaluation. Recommends solutions and engages in continuing education focused on the area of assignment and professional development. Provides orientation to newly licensed and less-experienced social workers and may serve as field instructor for graduate level social work students. Demonstrates knowledge and skill in the use of software applications, data management, and other computer systems necessary to provide PACT CRH SW care. Work Schedule: Full time, Monday-Friday. 8:00 am - 4:30 pm or 7:30am - 4:00pm depending on spoke site needs; Evenings, weekends, and holidays required as necessary based on the needs of the CRH / spoke sites.
