The Mental Health Clinical Pharmacist Specialist (MH­ CPS), coordinates, and directs various clinical programs within the Mental Health Service and those programs requested by other services. Covers direct and indirect patient care, patient teaching, quality assessment, staff education and consultation and is often in association with or in support of physicians, physician assistants. nurse practitioners, nurses, psychologists, social workers and others. The Clinical Pharmacist Specialist, Psychiatry coordinates, develops, manages and controls the pharmacy practice programs for Behavioral Health Service Line. This includes the clozapine program and psychiatric pharmacy student experiential learning. A. Routine Clinical Duties: Provides clinical pharmacy services to Mental Health Clinic(s) patients and other medical center patients by on-site consultations and direct patient management. Examples of such duties are as. follows: 1) Provides pharmaceutical services to focus on therapeutic drug monitoring. Analyze medication regimens to ensure the most effective, least .toxic. and most economical. treatment plan. Understands and applies the pharmacokinetic and pharmaco-dynamic: differences in the geriatric population. 2) Monitors medications, titrated and/or altered per all applicable pharmacy policies and procedures. 3) Interviews patients to ascertain significant past medical history and medication history. Evaluates medication ·regimens for appropriateness, drug interactions, therapeutic duplications, non-compliance and adverse drug events. Performs medication reconciliation. 4) Serves as a therapeutic consultant to the medical staff. 5) Documents all clinical activities, update problem lists, and updates medication allergy information in the medical record. 6) Orders laboratory tests or other tests as needed to monitor drug therapy effectiveness and safety. 7) Refers to appropriate services as needed via consultations. 8) Establishes and maintains effective interpersonal relations with others which reflect a cooperative attitude. 9) Responsible for the care of patients ranging in age from 17 years to > 100 years. B. Prescriptive Authority: Maintains authority to alter medications without co-signature of a physician. : C. Administration: 1) Supervises pharmacy students, pharmacy residents, and psychiatry residents on rotation. Ensures compliance With VA policies and procedures and any applicable laws or requirements from other regulatory agencies. 2) Participates in quality assurance and medication use evaluation programs for psychotropic medications. 3) Serves as a representative of Pharmacy Service on assigned committees and subcommittees. 4) Promotes cost effectiveness of the formulary. Serves as a consultant for non­formulary-requests for psychotropic medications. Advises on proposed non-formulary criteria for psychotropic medications. D. Interpersonal Relationships: 1) Serves as a liaison between Mental Health and the Pharmacy Service, assist in solving problems, clarifying orders, improving processes. and. communications to improve inter-service relationships. In contacts with patients, relatives, staff members, demonstrates a concerned, compassionate and professional demeanor to enhance the image of pharmacy, the medical center and the Veterans Health Administration. 2. Works with difficult situations involving all levels of contacts (patient, nurse; physician, pharmacy staff, etc.) including the ability to identify key elements of a situation and weigh the relative importance of issues, and cope with the dynamics of changing situations to maintain constructive relationships as well as response to situations that may change rapidly. 3. Responds to multiple problems or situations, simultaneously with limited time and resources and successfully resolve clinical care needs.
