This individual functions as a social worker responsible for the implementation of the Community Transition Center (CTC) within the Community Based Care Service Line. The CTC is a Psychosocial Rehabilitation and Recovery Center designed to assist Veterans with serious mental illness to develop the skills needed to successfully re-integrate to their community. This position is at the full performance level for Social Work. Social workers at this level are licensed or certified to independently practice social work. The incumbent is required to exercise independent judgment and decision making in the formulation of the recovery plan to facilitate Veteran's subsequent successful transition to community living. The incumbent will utilize primarily a group approach in assisting qualified Veterans in developing the skills to improve the quality of their life within the community. The incumbent will utilize a variety of individual approaches to provide guidance in the recovery process. The incumbent must have extensive skills in utilizing VA and community resources. Additional responsibilities include but are not limited to the following: Interprets VA's guidelines and eligibility requirements, and completes a comprehensive biopsychosocial history and assessment. Provide complex clinical services to clients with serious problems on a regular and continuing basis. Participate in program planning as well as in the development and facilitation of psychosocial rehabilitation groups based on the recovery model. Develop appropriate recovery plans. Using known available resources and the initial assessment of the Veteran's likelihood to accept differing types of assistance, makes initial and continuing decisions regarding the agency's services and referral. Ongoing assessment of the Veteran's progress and case management, as well as assuring that the Veteran is placed at the appropriate level of care. Perform administrative functions such as actively participating in clinic meetings; maintains accurate records of his/her clinical interactions; and assume responsibility for the implementation of programs he/she coordinates.
