The Acute Care Medical / Surgical Social Worker provides psychosocial and case management services to eligible veterans and their family members to meet biological, psychological, social and environmental needs on the Acute Care Medical / Surgical Units. The Social Worker manages, plans, implements, monitors, and evaluates the delivery of services within the Acute Care Medical / Surgical Units including the inpatient bed sections at the Medical Center for medical and surgical patients. The Social Worker in this position functions within the Acute Care Medical/ Surgical Units. Additional duties Include but are not limited to: A. Identifying behaviors or symptoms of abuse, neglect or exploitation B. Providing social work case management C. Acting as an advocate with appropriate VA and community service providers/agencies when it serves the best interest of the Veteran and family members/caregiver D. Assessing the psychosocial functioning and needs of Veterans and their family members identifying the Veteran's strengths, weaknesses, coping skills and psychosocial acuity, in collaboration with the Veteran, family, and interdisciplinary treatment teams E. Maintaining a current network of internal and external resources to educate the Veteran and/or family members/caregivers and assist with the appropriate referrals F. The incumbent is responsible for completing the following duties/responsibilities G. Serves as the social work provider for inpatient veterans in the Acute Care Medical / Surgical Units. H. Maintain accessibility to clients and ensure coverage during scheduled absences is communicated through the Social Work office to treatment teams. I. Identifying behaviors or symptoms of abuse, neglect or exploitation; J. Providing education on advance directives and advanced care planning; K. Providing social work case management L. Acting as an advocate with appropriate VA and community service providers/agencies when it serves the best interest of the Veteran and family members/caregiver M. Assessing the psychosocial functioning and needs of Veterans and their family members identifying the Veteran's strengths, weaknesses, coping skills and psychosocial acuity, in collaboration with the Veteran, family, and interdisciplinary treatment teams N. Maintaining a current network of internal and external resources to educate the Veteran and/or family members/caregivers and assist with the appropriate referrals O. The incumbent will participate in discharge planning with other disciplines and will be responsible for ensuring that discharge plans are executed in a manner that is timely and appropriate P. The incumbent will provide individual, family and group counseling as appropriate and utilize consultation and supervision. Q. The incumbent will act as a consultant to other team/staff members R. The incumbent will provide consultation/education to medical residents on social work practice and VA policy and procedure. S. The incumbent will address Advance Directives, Guardianships, Commitments and Organ Donation in accordance with the standards of their work area. T. The incumbent will coordinate treatment and communicate with the care lines to facilitate continuity of care for veterans. U. The incumbent will assist patients and their significant others with coping and dealing with the loss and grief experiences in disability, terminal illness and death. V. The incumbent will document social work interventions and activities in patient's clinical record utilizing CPRS and ensure appropriate hand off when transferring of patient care. W. The incumbent will provide a full range of social work services and interventions on the weekend during their regularily scheduled tour of duty to include coverage for social work issues in the emergency department and on the inpatient wards. X. The incumbent will provide coverage for advance directives across the inpatient wards on the weekend as well as triage social work consults printed to the social work office and address those consults which are deemed a priority. Y. The incumbent is responsible for coordinating and participating in the Admission Process if working in the CLC. Z. The incumbent will complete other duties as assigned by the Chief of Social Work & Chaplain Service or direct supervisor.
