The Veterans Justice Outreach (VJO) Coordinator is a Senior Social Worker whose primary responsibilities are to provide intensive case management services, continuous quality involvement activities, evaluation/consultation, psychosocial assessments, and referrals to Various community agencies for veterans who are incarcerated. The VJO is administratively responsible for clinical programing and daily operation of the program, as well as manages data related to the program. The VJO is an integral member of the Homeless Program and works in conjunction with the Homeless Program Coordinator and the Mental Health Social Work Supervisor to coordinate administrative, patient, and personnel activities for this program. Other duties include, but not limited to: Develop treatment plans for Veterans. Serves to help Veterans when interacting with government and community agencies. Provides referrals for both VA and non VA programs. Conducts individual and family clinical intervention using professional judgement. Documents progress notes within an electronic system. Collaborates with county probation officers, state attorney's office public defenders, and hired attorneys to develop post-release plan and/or alternatives, as well as, healthcare facilities. Meets with county jail administrators and staff on regular basis. Attends correctional institution meetings as a VA representative. Identifies and interviews incarcerated veterans to conduct pre-release needs assessment and appropriate post- release referrals. For this position you must have a current valid Driver's License. Work Schedule: Monday- Friday (Tours may run into the evening shift due to operational needs) Financial Disclosure Report: Not required
