| This position is eligible for the Education Debt Reduction Program (EDRP), a student loan payment reimbursement program. You must meet specific individual eligibility requirements in accordance with VHA policy and submit your EDRP application within four months of appointment. Approval, award amount (up to $200,000) and eligibility period (one to five years) are determined by the VHA Education Loan Repayment Services program office after complete review of the EDRP application. Learn more Duties include, but are not limited to, the following: Provides complex clinical services to clients with serious psychological problems, including suicide prevention, crisis intervention, short/long-term treatment, emergency food and shelter location, legal referral, employment, structured placement, financial matters, socialization and any problems encountered in daily living; Completes annual psychosocial assessments; Makes routine home visits for case management &/or coordination of care; Conducts community visits to Intensive Community Mental Health Recovery (ICMHR) clients; Collaborates with community residential care homes, where MHICM Veterans reside, to facilitate community living and provide support services for the CRC homes; Transports Veterans to medical appointments, as well as job sites, and to other social/community resources; Responsible for ongoing assessments and adjustments of treatment plans/progress, as well as developing discharge plans; Works in conjunction with inpatient Mental Health social workers by providing assistance with difficult discharges; Provides continuity of care by following up with clients discharged from mental health inpatient care; Provides outreach services to clients; Engages in community networking, which may include coordination with lawyers, residential care home operators, physicians, clergymen, and representatives of other disciplines; Performs other related assigned duties, as requested.
