| This co-located collaborative care position is administratively held within the Mental Health Service Line, programmatically assigned to the Primary Care-Mental Health Integration (PCMHI) Program, located within primary care. The PCMHI Licensed Professional Mental Health Counselor (LPMHC) functions as part of a multidisciplinary primary care team, and delivers brief consultation-based services to Veterans and primary care providers. The focus is on general service delivery for a wide range of concerns, and resolving problems within the primary care service context. Behavioral Health visits are brief (generally 20-30 minutes), limited in number (1-6 visits), and are provided in the primary care practice area, structured so that the patient views meeting with the behavioral health provider as a routine primary care service. Position functions within fast-paced primary care teams and can expect to consult with 8-10 patients per day with the primary goals of assisting primary care with identification, treatment, and management of mental health and behavioral medicine conditions in the enrolled population. The incumbent also functions as a primary referral source for more acute patients into specialty services for Behavioral Health. The Senior Social Worker is capable of practicing at the advanced level and has specialized knowledge of practice typically related to a particular diagnosis or population. Case consultation with the supervisor is generally limited to only the most complex cases seen in the clinic. Incumbent should have at least 5 years in an area of specialized counseling practice and licensed or hold certification in a state at the independent practice level. The PCMHI Social Worker provides functional assessment, triage, intervention, education and consultative services regarding a wide range of mental health and behavioral medicine concerns, on referral from primary care providers and allied PACT members. The PCMHI Social Worker uses population based practice management strategies and maintains competences in the following core domains. PCMHI Clinical Practice Management: Conduct functional assessments, based on 30 minute appointments and will triage/refer to specialty care services as appropriate Applies principles of population based care (defines roles and identifies problems rapidly) - 75 percent of all appointments are 30 minutes (will vary by setting) - Uses brief assessments routinely (clinical reminders, PHQ-9, PCL, Audit C, etc.); conducts suicide assessment and safety planning for high risk veterans - Able to assist in accessing care for Veterans who are in need of psychiatric stabilization - Initial consultation appointment are based on functional assessments and treatment focuses on functional outcomes - Maintains daily open access slots - Next available "scheduled" appointment is within two weeks - Provides brief mental health and behavioral interventions (e.g., 1-6 session for broad range of concerns) - Uses time-limited, evidence-based interventions that are simple and concrete - Teaches self-management home-based practice interventions - Uses intermittent visit strategy and flexible patient contact strategies - Triages appropriately and efficiently to specialty mental health services - Provides behavioral health intervention (including but not limited) to: Stress management, substance abuse/dependence, sleep hygiene, relaxation training, anger management. They provide direct specialty services themselves in several areas of posttraumatic stress disorder, and depression management. Other interventions will be provided as clinically appropriate, based on patient need. - Develops treatment plans in collaboration with the veteran/family and with the interdisciplinary treatment team; establishes and maintains effective therapeutic relationships with veterans and their families experiencing a variety of psychiatric, medical and social problems PCMHI Group Clinical Practice Management Conducts group therapy with 6-12 patients for 60-90 minutes groups addressing posttraumatic stress disorder, anger management, and other mental health conditions Group are conducted as close ended group 6-12 sessions only Patient Care Management Overview Incumbent is able to provide advanced case management services and demonstrates knowledge of local, state and other VA programs and resources Has general knowledge of veteran's benefits and services relating to special programs, service connected compensation, and nonservice connected pension Facilitates referrals based upon veterans needs and eligibility Work Schedule: Monday through Friday, 8:00-4:30 with alternative 40-hour tours/work weeks considered but in no way guaranteed-based on the needs of the clinic.
