The Primary Purpose of this position is: Fort Worth Outpatient Clinic Psychiatric Outpatient Team in an intensive case management, Intensive and evidence-based psychotherapy program for Veterans living with mental health illness, severe mental health illness and mental health dual diagnoses. Fort Worth Outpatient Clinic Mental Health Clinic Psychiatric Outpatient Team is an interdisciplinary Behavioral Health Team-Base Care Program (BHIP). The Senior Social Worker supports the Fort Worth Outpatient Clinic, Mental Health Clinic, Psychiatric Outpatient Team, BHIP Program by providing intensive case management, psychotherapy engagement and ensuring continuity of care. The Senior Social Worker provides on-going use of case management and psychotherapy interventions to promote sustained mental health stabilization. The Senior Social Worker is an expert at the community mental health resources and available to assist all Fort Worth Outpatient Clinic Mental Health Clinic Psychiatric Outpatient Team staff and Veterans who are receiving care from the Fort Worth Outpatient Clinic Mental Health Clinic Psychiatric Outpatient Team by linking them to supportive resources found within Veterans Affairs and the community at large. The Senior Social Worker will function as the liaison between the Veteran and various needed resources to ensure appropriate coordination of care. The Senior Social Worker provides psychotherapy, including evidence-based psychotherapy in which the provider is certified to the Veteran in an individual or group setting. The Senior Social Worker will perform specialized treatment of complex physical or mental illness as it relates to mental health illness, severe mental health illness and comorbid substance use disorders. The Senior Social Worker will incorporate complex multiple causation in differential diagnosis and treatment of Veteran patients, including making psychosocial and psychiatric diagnoses within approved scope of practice. The Senior Social Worker has the ability to expand clinical knowledge in the profession, demonstrating innovation in the creation of new models of psychosocial assessment or intervention to identify and address specialized clinical needs of mental health, severe mental health, substance use disorders and comorbid. Ability to write policies, procedures, and/or practice guidelines pertaining to the specialty population or specialty treatment program. The Senior Social Worker will develop and implement methods for measuring effectiveness of social work practice and services, utilizing outcome evaluations and use of evidence-based treat modalities that are effective with Veterans who struggle with mental health illness, severe mental health illness, comorbid and substance use disorders along with medically and psychiatrically complex conditions. The Senior Social Worker also provides specialized assessments, and treatment planning, as well as mental health, severe mental health, substance use disorders and comorbid consultation, to other team members to provide interventions and strategies for engagement for difficult to engage clients. The Senior Social Worker independently conducts comprehensive assessments and develops treatments plans in collaboration with Veteran, family, and interdisciplinary treatment team. The Senior Social Worker will monitor e-consults, complete assessments, and make recommendations to the Fort Worth Outpatient Clinic Mental Health Clinic Psychiatric Outpatient Team BHIP Team in addition to provide direct clinical care to Veterans struggling with mental health illness, severe mental health illness, substance use disorders and comorbid concerns.
