Social Worker (HCHV Outreach)
This position is assigned to the Healthcare for Homeless Veterans (HCHV) Program at the Houston VAMC. The program places special emphasis on helping the homeless, chronically mentally ill veteran and is administered under the Veterans Care and Transitional Service (VCTS). HCHV collaborates with the Community Resource and Referral Center (CRRC) within Michael E. DeBakey VA Medical System and other community agencies within the Continuum of Care. Incumbent is required to meet with Veterans in the community, often in places not meant for human habitation: on the streets, under bridges, in encampments, etc. Incumbent is on his/her feet for long periods of time, walking around the community, or driving around the Houston area to meet with Homeless Veterans.
