• The Social Worker is responsible for the assessment, planning, implementation, coordination, monitoring and evaluation of the patient's plan of care from admission to post-discharge. The Social Worker utilizes critical thinking skills, and the principles of case management to coordinate and implement a discharge plan that meets the patient's needs and ensures a seamless, effective and efficient transition of care across the continuum.

• Education/Qualification
  • Current Licensure in Social Worker (LGSW/LCSW/LICSW) in the District of Columbia.
  • Master's degree in Social Work
  • Attendance and Punctuality - Demonstrates the ability to arrive at work site on-time, limits the number of absences, provides adequate notice to supervisor for time off, takes lunch periods and breaks within prescribed times.
  • Mandatory training, testing, and competencies completed on time - Competency Training; Services Excellence Training; Annual Physical.
  • Three years experience in acute care or other related healthcare field is preferred
  • Case Management Certification preferred.
  • Previous experience in case management and/or discharge planning is strongly preferred.
