This Social Work position is located at VA Western New York Healthcare System. The incumbent is assigned to Outpatient Primary Care, Patient Aligned Care Team (PACT), is a part of and works in collaboration with the extended Interdisciplinary PACT. The PACT Social Worker is responsible for delivery of care, referrals for treatment services and case management services. The incumbent is assigned to work with a variety of patients including psychiatric, medical/surgical patients and out-patients. Applicants pending the completion of educational or certification/licensure requirements may be referred and tentatively selected but may not be hired until all requirements are met.
