This position is for a clinical social worker in Primary Care Mental Health Integration. The primary responsibilities of this position are to provide integrated mental health services in the Primary Care setting. The services will be based on a blended model which includes co-located collaborative care and care management. These services will address issues related to mental illness, substance use and health management. BASIC REQUIREMENTS a. Citizenship. Be a citizen of the United States. b. Education. Have a master's degree in social work from a school of social work fully accredited by the Council on Social Work Education (CSWE). Graduates of schools of social work that are in candidacy status do not meet this requirement until the school of social work is fully accredited. A doctoral de
