The Community-based Social Worker within the CBEC section of Social Work Service, provides social work services to Veterans receiving health care at the Veterans Health Indiana VA. The position is a split time dedicated to providing community based assessments with referrals to our Veterans involved in the CHF/COPD (Congestive Heart failure/Chronic Obstructive Pulmonary Disorder) initiative and Quality Oversight, dedicated to our community partners providing in home services. Applicants pending the completion of educational or certification/licensure requirements may be referred and tentatively selected but may not be hired until all requirements are met.
