JOB SUMMARY: This position is responsible for taking the lead and making recommendations to the VP of Health Services in the development of a strategy for the most effective use of Community Health Workers with CHC. Be responsible for identifying key services CHC needs to be involved and identification of where the majority of our members could most benefit. Will be responsible for training, management and day to day supervision of the Community Health Workers and be a key member of the Health Services Management Team. MINIMUM QUALIFICATIONS: Education/Specialized Training/Licensure: Masters in Social Work/LCSW or LMSW-AP, Case Management Certification desirable Work Experience (Years and Area): 5 years work experience, 2 year social work
