This Social Worker is assigned to the Primary Care - Mental Health Integration (PC-MHI) Program with responsibilities to address the psychosocial needs of eligible veterans. The incumbent is responsible for a broad range of skills and services including completing psychosocial history and assessments, individual, couples and group counseling, case management, as well as the coordination of treatment plans for veterans to ensure seamless transition of care.
