Position Summary: Case Mgmt Supv (D) uses a combination of case management nursing, healthcare and social work to assess all possibilities of a patients situation. They are responsible for collaboration with the healthcare team, in the development, planning, coordination and administration of the activities of Utilization Review and Resource Management. Case Mgmt Supv (D) acts as an intermediary between patients, physicians and the hospital. Management includes, but is not limited to, daily review of patient census to determine medical appropriateness for admission, continued stay reviews, high risk screenings and comprehensive discharge assessment for use of ancillary services. Case Mgmt Supv (D) provides patients and familys advocacy support and accurate healthcare information for proper treatment and safe discharge plans.
Minimum Requirements: 5 years pertinent clinical experience in an acute care setting Clinical Case Management experience
Education/Licenses/Certifications: Bachelor's degree in Nursing, or obtained within three years from the initial date of hire Advanced Degree in Nursing (MSN, PhD, DNP, ARNP) preferredLicensed in the State of Washington as a Registered Nurse Certified Case Manager (CCM) by the Commission for Case Management Certification (CCMC) preferred.
