SUMMARY: The Medical Social Worker provides medical social services to patients in the home. The Social Worker provides services to patients and family, facilitating the adjustment of those individuals to the impact of illness, assisting with community resources. Initiates referral processes as appropriate. EXPERIENCE: One year acute care; home health experience preferred SPECIAL SKILLS: Knowledge of community resources, Medicare/Medicaid and JCAHO required OTHER: Must possess strong communication and interpersonal skills. Possess current drivers license and auto liability insurance. LICENSE/CERT: Required: LCSW (Licensed Clinical Social Worker) or LMSW (Licensed Master Social Worker) Preferred: ACSW (Academy of Certified Social Workers) EDUCATION: Required: Masters - Social Work (CSWE app
