This Position combines Veterans Justice Outreach and Health Care for ReEntry Veterans. The social worker conducts outreach in the catchment area's prisons, jails and courts completing psychosocial assessments and providing case management services. Works in conjunction with the HUDVASH, Community Employment Coordinator and Health Care for Homeless Veteran Programs to coordinate needs and services of this program with VA and non-VA providers assuring access and ongoing services as needed.
