Trainee will connect and manage patients&#8217; receiving MOUD in receiving behavioral health counseling and in connecting to social supports to retain patients in care. Trainee will collaborate with care givers inside and outside of the immediate care team. Trainee will be able to demonstrate clearly and effective communication with patients and immediate care team. Provide patient education and counseling covering health status, test results, disease processes, and discharge planning. Perform procedures. Work is typically performed in a clinical environment.
