Participate in weekly interdisciplinary team meetings and case conferences, as necessary, to assure appropriate care and service are provided to the patient. Maintain up to date knowledge of Medicare/Medicaid/JCAHO standards thru inservices and workshop attendance. Assesse the severity of patient symptoms, communicate to the physician and record significant findings, intervenes as appropriate. Document care and services provided in accordance with DHCH Policy and Procedures and regulatory standards. Complete documentation accurately, timely, concisely. Medication reconciliation and education to prevent adverse events and readmission to hospital Observe patient?s reactions to medications and reports significant incidents to the physicians. Communicate regularly with team members, caregivers, and physicians. Follow infection control standard precautions and uses personal protective equipment as required. Delegate and supervise care given by NCA?s/LPN?s. Develop plan of care for aides, assign care to aides based on the skills of the aide, the availability of the aide for patient care continuity, patient preference (when possible), and other considerations as determined by the patient?s care needs. Identify personal learning needs and implements corrective action: assists in planning and conducting staff development programs to improve DHCC effectiveness. Participate in own professional development by maintaining required competencies, identifying learning needs and seeking appropriate assistance or educational offerings. Implement agency policy, procedures and objectives. Work closely and collaboratively with agency billing staff to ensure accuracy of billing. Perform other duties as assigned. &#xa0; Home Health Case manage interdisciplinary team of clinicians caring for patients in the home. Complete required OASIS Comprehensive assessments at admission, recertification, Resumption of care and discharge.&#xa0; This assessment determines patient reimbursement for Medicare and other episodic payors and also results in publicly reported patient outcomes and additional payment impacts related to value based purchasing Work with interdisciplinary team and patient providers to prevent re-hospitalization of the patient. Be aware of patient insurance requirements and assures authorization for all services have been obtained. Duties The position leads a team comprised of Peer Specialists and may lead Without Compensation (WOC) peer support employees - within a common supervisory structure - to provide services - integrate processes - and meet goals of peer support service programming at the facility As the leader of a team of peer support staff - the position facilitates - coordinates - coaches - and problem solve with the members of the peer support staff team The position also acts as the recognized liaison to and from the peer support staff team and the supervisor The work of the position enables the peer support provider team as a whole to deliver improved quality - quantity - and timely services to Veterans Duties include - but not limited to the following: Ensures that the organization&#39;s strategic plan - mission - vision - and values are communicated to the peer support staff they lead and integrated into the team&#39;s strategies - goals - objectives - work plans - products - and services Articulates and communicates to the peer support staff they lead the assignments - projects - problems to be solved - actionable events - milestones - and/or program issues under review as well as the deadlines and time frames for completion The position communicates the supervisor&#39;s instructions to the team Coaches the peer support staff they lead in the selection and application of appropriate problem-solving methods and techniques - provides advice on work methods - practices and procedures - and assists the team and/or individual members in identifying the parameters of viable solutions Maintains awareness of the status and progress of peer support staff&#39;s work and makes day-to-day adjustments to ensure their work aligns with program goals and processes Leads the team in identifying - distributing - and balancing workload and tasks in accordance with established workflow - skill level and/or occupational specialization makes adjustments to accomplish the workload in accordance with established priorities to ensure timely accomplishment of assigned team tasks and ensures that each team member has an integral role in developing the final team products and services Identifies training needs and assists in the development of training for peer specialists and Veterans seeking to become peer specialists Serves as a coach - facilitator and/or negotiator in coordinating team initiatives and in consensus building activities among team members Maintains working knowledge of treatments and services available for the purpose of appropriate referrals Represents the team in dealings with the supervisor for the purpose of obtaining resources (e.g. - computer hardware and software - use of overtime or compensatory time) - and securing needed information or decisions from the supervisor on major work problems and issues that arise Participates in the service - training - consultation - and other professional activities of VA health care services and functions as a fully vested member of the multidisciplinary team Uses personal recovery experiences to model coping techniques and recovery tools to coach Veterans on relevant skills needed for self-management of their health condition(s) and pursuit of their personal wellness Observes behaviors that might indicate difficulty adapting or responding to treatment (e.g. - missed assessment appointments - failure to attend or maintain abstinence - risk to self or others - disruptive behavior) - completes appropriate documentation - and reports concerns to the treatment team in a timely manner Uses ongoing individual and group meetings to teach Veterans how to identify and combat negative self-talk and overcome fears by providing a forum that allows Veteran to share their experiences As necessary - handles crisis interventions for Veterans and addresses other emergent situations without the benefit of specific instructions Informs Veterans about VA and community supports and resources - including how to effectively utilize them in the recovery and wellness process Maintains a working knowledge of current trends and developments in holistic approaches in wellness and recovery by reading books - journals - and other relevant materials Performs other related duties as assigned Work Schedule: 8:00am - 4:30pm - Mon -Fri Virtual: This is not a virtual position Position Description/PD#: Lead Peer Specialist/PD99338S Relocation/Recruitment Incentives: Not Authorized Permanent Change of Station (PCS): Not Authorized</description>

Summary: With oversight and direction from the Program Director, the Graduate Medical Education (GME) Program Coordinator is a member of the leadership team of the program, is responsible for the coordination and administration of the accredited/non- accredited residency/fellowship training program, and is critical to the success of the program. Responsibilities: 1.Serve as the lead administrative person for the GME residency/fellowship program. Manage the daily operations and activities of the GME residency/fellowship program(s). Assist the GME Program Director and/or other levels of GME Program Coordinators or Administrators in the design, implementation and management of the administrative/academic operations of the program. Provide guidance to residents/fellows on program and GME policies, and non-clinical aspects of the program. 3. Coordinate accreditation activities including ACGME Site Visits, Self-Studies, Internal Reviews, annual program reviews, etc. Develop understanding of ACGME policies and procedures, including familiarity with the Common Program Requirements as well as Specialty-Specific requirements. Ensure GME and departmental and program policies and procedures are communicated/available to the GME Program Director, faculty, residents/fellows, and staff. 4.&#xa0; Assist with resident/fellow recruitment and selection processand oversees the appointment process for incoming trainees in coordination with policies and procedures provided by the GME Office. 5. Assist the PD and faculty in planning program quality improvement projects. Prepare Program Letters of Agreement (in coordination with the GME Office) with participating institutions and training sites. Ensure agreements are revised as needed. Track, document, and report on educational activities for residents, for the purpose of meeting board certification requirements, meeting program accreditation requirements, and complying with the policies and procedures of the Office of Graduate Medical Education (GME). Utilize systems and reporting to inform the GME Program Director of program compliance with organizations such as ACGME, AMA, NRMP, and any other program-level related organizations. Utilize the RMS to record, maintain and report on all aspects of the resident?s/fellow?s training experience(s), including demographics, scholarly activity (including faculty), procedure certification, performance/semi/summative evaluations, block/shift/clinic schedule, and duty hours. 6. Maintain close working relationships with the Office of GME to remain compliant with all necessary requirements for all residents/fellows, visiting residents, and faculty.
