Overview
Atlantic Health System is currently seeking a Full-time Social Worker, within the Atlantic Homecare & Hospice division to be available to work 8a-4p and week-end hours as needed.
Responsibilities
Essential Functions/Responsibilities of Position:
Atlantic Visiting Nurse is looking for a dynamic and team oriented individual to become part of their team. This person will:
Perform comprehensive psychosocial assessments
Identify behaviors and issues that affect coping at the end of life
Identify issues that may have an impact on the safety of patients and families Identify the availability of support systems to meet patient/family goals
Identify bereavement risks and communicates these appropriately
Embraces a team approach and collaborates with other disciplines in the development of the plan of care
Counsel patients and families in strategies to cope with issues of loss and terminal illness and participate with other disciplines in family conferences
Provide referrals to appropriate community services
Facilitate movement between levels of hospice care
Maintains high standards of professional practice
Maintain current knowledge of community resources and eligibility requirements
Maintain knowledge of hospice Medicare benefit and other insurance coverage
Participate in the agency's Performance Improvement program
Participate in utilization review activities
Participate in community education and outreach
Qualifications
Minimum Education Required:
- Master's in Social Work from an accredited school of social work
Minimum Experience Required:
- NJ Social Work License LSW or LCSW required.
QUALFICATIONS:
1-2 years' experience in hospice/ palliative or geriatric care preferred
Expertise in community resources
Excellent customer service and communication skills
Knowledgeable regarding Hospice Conditions of Participation preferred
